The 3rd Quarter of 2006 ends September 30, 2006.
Visit the STAR (Setting Targets - Achieving Results) website www.nhqi-star.org and set your Target goal for your Quality Improvement programs for the Quality Measures: Restraint Elimination, High Risk Pressure Ulcer Prevention and Management as well as Pain and Depression Management for the 3rd Quarter of 2006.
The STAR site trends your Nursing Home's Quality Measure (QM) data, derived from your MDS submissions, as posted on Nursing Home Compare (www.medicare.gov/NHCompare ) for these 4 Quality Measures. The STAR site allows you to set Targets goals, based on this data, to guide your Continuous Quality Improvement projects.
The STAR site was introduced last year at this time. At least annually, each Nursing Home's Quality Improvement Team should review their Quality Improvement Goals and evaluate their progress towards achieving these goals.
Print the STAR graphs and share the graphs with all staff. Review your programs to achieve the goals. Highlight your successes and identify areas needing new strategies for improvement. Then work with facility interdisciplinary teams, review current policies and processes to identify what may need to be changed or identify processes not working. Encourage staff to identify problems with the process and to develop alternative methods to deliver the needed resident care.
A target must be reset for each of the measures as of September 2006 to show on your graph for the 3rd quarter. You may continue your present target goal, but resetting it, documents the date that you have reviewed your targets. For measures where you are low or may have consistently achieved " 0 " such as restraints, please reset the target for "0", indicating that your facility is continuing the goal of zero restraints.
