discharge instructions sheet

If a patient has a discharge instructions sheet in the chart that has the patients addressogragh on it and the signature portion for the MD, Nurse, and patient are blank, is it assumed to have been given even if there is no documentation in the chart that the instructions were explained to the patient? Do you mark yes to the different fields?
