This information on Negative Pressure Wound Therapy (a.k.a. Vacuum-Assisted Closure) for Treatment of Pressure Ulcers was shared by the National Nursing Home Improvement Collaborative. Some facilities have asked about this procedure, some have used it. Please contribute your experiences, opinions and/or questions on this or any other treatment or topic.

This report is a simple tool developed to assist a facility with small “tests of change” in their efforts with pressure ulcer prevention and treatment. This is a more focused tool utilized to summarize results of separate audits in areas such as risk assessments, body checks, support surfaces and dietary interventions.



As with the other reports, this report also generates bar graphs on the monthly results.



In addition, there is an area on this report to detail specific process changes you may be tracking to calculate “% of compliance” with the specific process change(s) and provide you with a monthly trending graph of the ongoing results.



Again, this is meant for “small sample” efforts.... it may be a specific assignment that you may want to track and test a change. This method is recommended to allow you to test ideas, evaluate results, and spread your successes to a broader area.



This is the very essence of “Continuous Quality Improvement” (CQI) where the steps involve-

ü Plan- decide what you want to test/change

ü Do- carry out your test/change

ü Study- interpret the results / positive and negative

ü Act- make a decision, based on those results, to spread the change to a broader area or alter your plan for additional testing

This is our second month using your program. It's wonderful! It has shown where our weaknesses/strengths are and has enabled our facility to target the education needs of our staff. Thank you so much for creating it!!

We are using Pressure Ulcer Monthly Statistical Tracking Tool
...the one that enables us to track all our pressure ulcers and differentiate which were on admission etc...

If you have other programs as helpful as this I would love to try them!

Hi Cyndy,
I do have another Excel file that allows you to take the total stats off of the monthly tool you are using (from the summary sheet) and enter them easily on a monthly basis to get trending over an "extended" period of time... the tool covers a year. There are other items within this tool that you could choose to use or not... specifically, some audit questions for new admits on timely risk assessment, etc.... which would calculate (and graph) compliance to your processes. As well, there is an area to plug in monthly numbers from your QI/QM reports off of CASPER on some specific QI/QMs related to PU tracking and trending.... i.e. low risk bowel and bladder, incontinence, weight loss, dehydration, physical restraints, high risk, low risk and short stay pressure ulcers. These too would be graphed over an extended period of time which could help to uncover correlations.

There are many many graphs attached to this particular file. Some show # of pressure ulcers and others show %. Sometimes, when your numbers are low, % have less of an impact. The point is, the graphs are all there... part of the file... and automatically built as you enter your data. You decide which graphs to print.

I have attached 2 files.... one with sample data and one "blank" for your use.

Thanks, Dave, I look forward to trying this out. I am the chairperson for the skin/elimination committee so any bit of info I get is helpful! Like I said the pressure ulcer tracking form is wonderful...This additional form looks awesome also! Thanks for all your hard work to provide these tools to enable us to better coordinate care for our residents! I will be calling.
