I would like to receive feedback on the various home care agency's care delivery structures. We are looking to restructure the agency encompassing the supervisors, reviewers, intake, case managers, QI personnel, and education. Some of our ideas are the team concepts, or supervisors having an assistant so far and look forward to comments, thanks Norma Swartout, RN QI Coordinator.:D

What a task you all have ahead of you! In fact, it is too big to tackle in this forum; I’d love to chat with you about the care delivery structure at Community Health Center Home Care, in Johnstown. We have done very successful work helping other agencies tackle this very problem. For example, some organizations have used a team approach to care delivery or a service-delivery model of care. I am very familiar with both approaches to agency restructuring.
