ICD Implant Checklist

Attached is an ICD project update Administrative Memorandum along with an ICD “Implant Checklist”. This checklist may be used as a quick reference guide, to further assist physicians and hospitals in ensuring appropriate clinical indications are met, according to the current Medicare Coverage Guidelines. This checklist will also be revised and redistributed upon final issuance of the revised CMS Coverage Determinations.
