View Full Version : The CMS 9th Scope of Work IPRO Project for 2008

My facility has just agreed to participate in the CMS 9th Scope of Work project which is targeting high risk residents with pressure ulcers. IPRO will provide resources to "Identify gaps in care delivery systems, Implementing processess to improve efficacy of care, Enhancing patient-centered processess, and, developing a nursing home culture focused on safety, quality and satisfaction."
I would like to hear from any and all interested people out there, to start a discussion group on this new undertaking. I believe it is us, the "people in the trenches", who are out there facing the challenges of preventing skin breakdown in very compromised residents who have a lot to offer on this topic. So let's start talking! :D

Just a quick update on this project. We had our first meeting with Dan Yuricic to discuss the various IPRO tools that are available to assist in meeting the goals of lowering pressure ulcer prevalence as measured by the CMS QMI reports. Just by looking at how our ADLs are scored on the MDS, it became very obvious that we are undercoding our resident's abilities to our statistical detriment. The solution that can help was offered in the form of inservices and easy to follow documentation that can capture the true abilities of the residents as regards to minimal versus extensive assist for the MDS. The spreadsheet on pressure ulcers was a well thought out tool that we plan to implement as having more specific data then our current spreadsheet (for pressure ulcer site analysis) and as I always say, "why re-invent the wheel, when it's out there for you already". All in all I can say that we had a very productive meeting and I would highly recommend using this free service to any facility, not just those that have "problems", that would like to improve their pressure ulcer numbers. It was quite an eye opener to discover how that one small step of more accurately capturing the assitance we are providing to our residents...showing how much care we do provide for them...will give a more accurate statistical picture of our total quality of care. We were making ourselves look bad! And I'm sure we are not the only facility doing this. So check it out for yourself. There are also "free tools" posted on JENY for downloading to help in these matters as well. Anyone else participating in this project, please share your experiences and your thoughts...we're all in this together! :)

Under the 9th SOW IPRO Project for 2008, our facility has begun to address VTE Prevention and Treatment Project. A meeting was held at our facility on 9/19/08, including Pharmacy, Nursing, IS and Performance Improvement. A discussion was held to get a set of preprinted orders for VTE Prophylaxis. The form was to include: a VTE risk assessment, Contraindications to Prophylaxis, VTE risk in the hospitalized inpatient (low, moderate,high), a pharmacologic Prophylaxis
options, Mechanical Prophylaxis, lab work and a place for MD signature, date and time. We also discussed chart audit form for VTE Prophylaxis which would be completed by Pharmacy and PI study by Pharmacy. A discussion was also held regarding education for patients and family which would include a Lovenox packet and Coumadin teaching sheet. Dietary to be included related to food/drug interactions. We also discussed monitoring of lab work which a daily print out goes to Pharmacy including medication and labs. Our preprinted order form was taken to our Medical Staff Meeting and is now being presented to each individual department meeting for approval. With this in order, we feel we have a good handle on the program to this point. Would be glad to hear what others are doing on their project.
