The following information is from a 1.5 hour session provided by Terry Clemmer, MD, Director of Critical Care and Trauma, LDS Hospital, Intermountain Healthcare System. Dr. Clemmer is on the faculty of IHI's National High-Risk and Mortality IMPACT Project.
Dr. Clemmer has been gracious enough to share his insulin excel worksheets. Please note that these are for educational purposes. Each hospital would need to develop their own glucose/insulin tools, algorithms, worksheets, and dosing guidelines that best fit their patient populations and practices.
