2: Certified Diabetes Educator (Lake Success)

3: Compliance Reviewer (Lake Success)
This individual will participate in on-site assessments of teaching hospitals located in Long Island, the 5 Boroughs & Westchester by conducting surveillance activities, including interviewing residents. 4: Nephrology Nurse (Lake Success)
Responsibilities include: developing and implementing Quality Improvement programs for dialysis and transplant centers under the direction of the Project Manager; supporting the Medical Review Board under the direction of the Project Manager by providing creative insight into problem identification, measurement and resolution; responding to requests for information from patients and providers in the Network's service area. 5: Patient Services Coordinator (Lake Success)
This individual serves as the point of contact for inquiries related to care provided to dialysis and transplant patients in the New York area. Primarily responds to quality of care concerns from the community and promotes informed health care decision making and rehabilitation for individuals receiving treatment for End Stage Renal Disease (ESRD).
