We have developed a chronic disease registry using Excel. The intention of the disease registry is to present the physician with a method of tracking and evaluating their patient population with regard to four chronic disease conditions: CAD, DM, HF, & HTN. Additionally, these four conditions are supported with five prevention measures: breast cancer screening, colonoscopy screening, influenza and pneumonia vaccinations, and tobacco use counseling.
Please feel free to download and use the spreadsheet. As with all things DOQ-IT, it is free. We have also collected from the web, other QIO and medical sites, a selection of patient action plans.
