Here is an interesting article from April's AAFP Journal.

The spreadsheet is a good way to start building a patient registry without the expense of an EHR. It can be modified or simply reconfigured for other chronic diseases such as CAD, Heart Failure, Hypertension or even preventative measures, such as cancer screening or smoking cessation.
